Job summary

BAYADA at BAYhealth is seeking a Registered Nurse to perform home health visits, assessments, and admissions with adult patients in Dover, Camden, and Magnolia, Delaware. This office offers both full-time and part-time opportunities with flexible schedules.

As an RN, you will join a multidisciplinary team delivering rehabilitative and skilled nursing care at patients' homes, following care plans and documenting using a state-of-the-art EMR.

Qualifications

• Active Delaware RN license.
• Minimum of 1 year of recent clinical (medical/surgical) experience.
• Home health experience preferred but not required.

Responsibilities

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients in designated geographic territories (Dover, Camden, Magnolia).
• Administer medications, provide wound care, treatments and procedures.
• Monitor client conditions and report changes to supervisor.
• Document doctors' orders and care observations accurately.
• Perform client assessments and coordinate case management.
• Document observations and evaluations in the EMR.
• Travel to cases as assigned.
