Job summary

BAYADA Home Health Care is seeking a Case Manager Registered Nurse for a full-time or part-time opportunity to serve adult patients in their homes across Sussex County, DE, including Seaford, Milford, and Laurel area.

As an RN, you will join a multidisciplinary team delivering rehabilitative and skilled nursing care to enable patients to remain at home. Qualifications include DE RN licensure, 1 year clinical experience, and travel readiness, with EMR proficiency and strong interpersonal skills.

Qualifications

• DE Registered Nurse license
• Minimum of 1 year of clinical (medical/surgical) experience
• Home care experience preferred but not required
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned
• Strong interpersonal skills and good computer skills; EMR experience preferred

Responsibilities

• Follow designated care plans for patients in their homes
• Make home visits to clients in designated geographic territories
• Administer medications, wound care, treatments and procedures
• Monitor clients' conditions and report changes to the manager
• Document doctors' orders and interventions accurately
• Perform client assessments and coordinate case management
• Document observations and evaluations using electronic records
