Job summary

Nurses at Home is seeking a full-time RN Clinical Case Manager to provide high-quality care on the road in Connecticut. The role emphasizes clinical first, with strong patient interaction and OASIS proficiency. Ideal candidates have home health experience and an active RN license.

Qualifications

• OASIS proficiency preferred for full-time RN clinical case managers.
• Bachelor's degree preferred; RN license required.
• 1 year home health experience preferred; strong patient-facing communication essential.

Responsibilities

• Provide RN clinical case management for home health patients.
• Coordinate care plans and communicate with families and clinicians.
• Ensure compliance with care standards and documentation requirements.
