Boost Home Healthcare in Fort Collins, Colorado is seeking an experienced Registered Nurse Case Manager to oversee in-home client care and lead a small team of caregivers. You will assess needs, develop plans, and coordinate with families, physicians, and community partners to ensure high-quality, client-centered services.

This full-time role offers a competitive hourly pay range and a flexible schedule with weekday days and occasional weekend rotation.

Qualifications

• Active, unencumbered RN license in Colorado (required).
• At least 1 year of home care or home health nursing experience.
• Experience with case management and care coordination.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
• OASIS documentation experience preferred.
• Strong clinical judgment, organization and communication skills; client-centered care.

Responsibilities

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments of each client’s physical, cognitive, psychosocial and home safety needs.
• Care plan development: Create and update individualized plans of care and make sure caregivers understand and follow them.
• Caseload management: Oversee a caseload of home care clients, tracking changes in condition and adjusting services as needs change.
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed.
• Documentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines.
• On-call support: Help resolve client and caregiver concerns quickly and professionally.
