BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health case management in Gaithersburg, Maryland. This full-time role supports a multi-disciplinary team delivering skilled nursing and rehabilitative care to clients at home, with prior home health experience required.

Qualifications

• Maryland nursing license in good standing.
• Graduation from an accredited nursing program (school transcript or diploma).
• At least one year of clinical experience with adult/geriatric populations in relevant settings (acute care, home health, hospice, ICU, med-surg, sub-acute, or rehab).

Responsibilities

• Use head-to-toe assessment to identify subtle changes in patient condition.
• Apply experience with CHF, COPD, wound care, IV therapies, post-surgical care.
• Triage and manage urgent clinical situations in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
