Boost Home Healthcare - North Shore (MA) is seeking a compassionateRegistered Nurse to join as a Home Health Case Manager. You will manage a caseload, complete assessments, and collaborate with a multidisciplinary team to deliver high-quality care in patients' homes.

Qualifications

• Active RN license in state.
• At least 1 year home health or case-management experience preferred.
• Experience with EHR systems; Kantime/Axxess/HCHB/WellSky a plus.

Responsibilities

• Serve as primary contact for a caseload of home health patients and coordinate care.
• Perform OASIS assessments and develop care plans with the interdisciplinary team.
• Coordinate services with physicians, nurses, therapists, social workers, and aides to ensure timely care.
• Monitor patient progress, update plans, and communicate with families as needed.
• Maintain accurate clinical documentation per Medicare guidelines and agency policy.
• Educate and support patients and families to promote independence and wellness.
