Boost Home Healthcare in Brookline, MA is seeking a compassionate Registered Nurse to join our Home Health Case Manager team. You will be the primary contact for a caseload of home health patients, coordinating services and ensuring care is delivered on time.

You will conduct OASIS assessments, develop individualized plans with the interdisciplinary team, and monitor patient progress. Strong communication and independent work are essential in our supportive, autonomy-focused environment.

Qualifications

Active RN license required.
At least 1 year of home health or case management preferred.
Strong critical thinking, communication, and time-management skills.
Ability to work independently and manage a caseload.

Responsibilities

Manage the Care Journey: Serve as the primary contact for a caseload of home health patients, coordinating services.
Conduct Comprehensive Assessments: Complete OASIS assessments and develop care plans with the interdisciplinary team.
Coordinate Services: Collaborate with physicians, nurses, therapists, social workers, and home health aides for timely care.
Monitor and Evaluate: Track patient progress, update care plans, and communicate with families.
Documentation: Maintain accurate clinical records per Medicare guidelines and agency policy.
Advocate for the Patient: Provide education and support to promote independence and wellness.
