Qualifications

Responsibilities

• Make decisions and take appropriate action with supervision as needed.
• Coordinate patient care with providers, nurses, therapists, social workers, aides and families.
• Develop and adjust the plan of care, monitor progress, and document in the chart.
• Ensure HIPAA/privacy compliance and maintain patient confidentiality.
• Utilize resources to minimize costs and ensure quality patient care.
