Job summary

Optum is seeking a Patient Care Manager in Denver, CO to supervise and coordinate clinical services for home health patients. You will lead an interdisciplinary team to ensure high-quality, physician-guided care compliant with state and federal laws.

Responsibilities include directing clinical services, managing referrals, coordinating benefits and approvals, and ensuring care plans are followed. Strong leadership and RN licensure are required for this role.

Qualifications

• Current RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provides clinical services within the scope of practice, as defined by state laws governing nursing and the plan of care, in coordination with the health care team.
• Receives referrals and assigns clinicians/therapists for timely patient evaluation and SOC visits.
• Coordinates patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensures patient needs are assessed and care meets eligibility criteria and physician orders.
• Manages and documents calls from physicians, clinicians, patients, and referral sources; communicates updates to clinicians.
• Reviews assessments and plans of care daily and consults clinicians with recommendations.
