Job summary

Elite Home Health, part of LHC Group within the Optum family, is seeking a Registered Nurse to provide and direct nursing care to patients in their homes across Fort Smith, AR. You will coordinate total care with physicians and the health care team while adhering to laws, regulations, and policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with physician and health care professionals.
• Supervises home health aides and licensed staff, providing instruction and task assignment per regulations.
• Performs supervisory visits and ensures high-quality clinical services per plan of care.
• Completes assessments and medication reconciliation, documents visits per policy and payer requirements.
• Maintains timely communication with physicians, patients, families, and agency staff to ensure quality care.
• Participates in on-call and weekend rotations and supports survey readiness and performance improvement.
