Job summary

At Home Healthcare, a part of LHC Group, a leading post-acute care partner for hospitals, physicians and families nationwide, is seeking a Registered Nurse in Home Health in Arizona.

You will provide and direct nursing care in patients’ homes, coordinate the plan of care with the physician and health care team, and ensure continuity of patient care by exchanging information with clinicians, patients, and families.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire; 1+ year home health experience is a plus.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and health care team.
• Supervises home health aides and LPNs; provides instruction and assigns tasks per state and federal regulations.
• Performs required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Communicates with physicians, patients, and families to ensure quality care and service excellence.
