Job summary

Traditional Home Health Care in Scranton, PA is seeking an experienced RN for a full-time position serving Scranton and surrounding areas.

You will assess patients, develop and adjust care plans in collaboration with physicians and families, and provide skilled nursing care, patient education, and timely documentation to support home health services. This role requires valid PA RN license, ability to travel to cases, and strong communication and organizational skills.

Qualifications

• RN license in Pennsylvania
• Driver's license required
• Ability to travel to case assignments
• Excellent assessment and documentation skills
• Ability to work independently with minimal supervision
• Strong interpersonal and communication skills
• Acute care and/or home health experience preferred

Responsibilities

• Admits patients to Traditional Home Health Care.
• Assesses patient status and develops plan of care in collaboration with physician, nursing supervisor, patient and family.
• Administers patient care, management and evaluation of the care plan, and patient teaching as ordered.
• Reports patient status changes to physician and nursing supervisor; revises plan of care as needed.
• Maintains meticulous documentation of interactions, care plans, and changes to plan of care.
• Acts as liaison with patients, therapists, nursing supervisor, and health care team; makes referrals as needed.
