Qualifications

• Active RN license in PA or New Grad with eligibility to take NCLEX.

Responsibilities

• Conduct Start of Care comprehensive assessments and timely OASIS completion.
• Draft care plans, reconcile patient medications, and ensure accurate orders for care.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide proper reports to the Case Manager.
• Provide health care instructions to patient/family per assessment and plan.
