Job summary

Hackensack Meridian Health is seeking a Registered Nurse for Home Health to provide telephonic case management, coordinate patient care, and support clinicians in delivering high-quality home-based services. You will work within a multidisciplinary team, perform assessments, and ensure effective care plans.

The role requires 2+ years RN experience in home care, strong communication, and solid computer skills.

Qualifications

Associate's Degree in Nursing or Diploma Certificate in Nursing.
Minimum of two (2) or more years RN experience.
Minimum of three (3) years certified home care experience.
Excellent written and verbal communication skills.
Proficient computer skills that include Google Suite and/or Microsoft Office platforms.

Responsibilities

Assists the clinical operation by providing telephonic case management and quality, patient satisfaction and process flow initiatives and reports to the Manager of Clinical Practice.
Collaborates with specialists, other physicians, and professionals to provide home health services to patients in need of skilled nursing care.
Coordinates care by ensuring effective communication and completion of treatments/procedures.
Assists client care coordinators in prioritizing patients in scheduling visits for case managers via chart/document review and triage.
Functions as the patient advocate in coordinating patient care and referrals to community resources as needed.
