Qualifications

• Graduate of an accredited nursing program with an active RN license.
• Minimum of 1 year of nursing experience; home health experience is a significant plus.
• Strong critical thinking, proven clinical expertise, and patient-centered care.
• Current CPR/BLS certification and valid driver’s license with reliable transportation.

Responsabilités

• Be a clinical leader delivering high-quality nursing care in patients' homes.
• Craft individualized care plans and adjust them as needed.
• Collaborate with physicians, therapists, and support staff for holistic care.
• Educate patients and families, providing crucial teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster teamwork.
• Utilize technology to document care and optimize delivery.
