CommonSpirit Health at Home is seeking a Registered Nurse to provide compassionate, patient-centered care in patients' homes across Kentucky. You will lead clinical care, design individualized plans, and collaborate with a multidisciplinary team to support recovery and independence.

Qualifications

• Active RN license in the state served.
• Minimum 1 year of nursing experience; Home Health experience is a plus.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Be a clinical leader in the patient's home environment, delivering high-quality, personalized nursing care.
• Develop and adjust comprehensive care plans to promote patient independence.
• Coordinate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing essential teaching and emotional support.

Responsibilities

As a Home Health RN, you will:

• Collaborate and coordinate. Work seamlessly with a multidisciplinary team - physicians, therapists, and support staff - to ensure holistic care.
• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
• Job Requirements*Qualifications:
• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
