Job summary

CommonSpirit Health at Home is seeking an experienced Home Health Registered Nurse to provide patient-centered care in patients' homes around Westminster, CO. You will lead clinical plans, coordinate with a multidisciplinary team, and educate families to promote independence and safety.

Qualifications

• Active RN license in the state served.
• Minimum of 1 year nursing experience; home health is a plus.
• Current CPR/BLS certification is required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader and deliver high-quality, personalized nursing care in patients' homes.
• Craft individualized care plans and adjust them to promote patient independence and well-being.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing essential teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
• Utilize technology to optimize care delivery and maintain precise documentation.

As a Home Health RN, You Will

Job Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

$36.51 - $59.00 /hour
