Choice Matters Home Healthcare is seeking a Home Health LPN to provide in-home nursing care in Jacksonville, FL and surrounding areas. The role supports flexible scheduling, uses AI-assisted documentation, and leverages WellSky EMR to streamline charting while prioritizing patient care.

We value experienced clinicians who communicate well, stay organized, and collaborate with a multidisciplinary team. Full-Time, Part-Time, and PRN options are available to fit your life and schedule.

Qualifications

• Active Florida LPN license required.
• Reliable transportation and valid driver's license.
• Excellent clinical, communication, and organizational abilities.

Responsibilities

• Provide skilled nursing care in patients' homes under RN/physician guidance.
• Administer treatments, med management, wound care, and education.
• Document changes and report to supervising RN promptly.
• Collaborate with care team including physicians and therapists.
• Maintain Medicare and agency documentation standards.

What You’ll Do

• Provide skilled nursing care in patients’ homes under the direction of the Registered Nurse and physician
• Perform ordered treatments, medication management, wound care, and patient education within the LPN scope of practice
• Observe, document, and report changes in patient condition to the supervising RN
• Collaborate with physicians, nurses, therapists, and other members of the care team
• Complete accurate and timely documentation while maintaining Medicare and agency standards

Qualifications

• Current Florida Licensed Practical Nurse (LPN) license
• Valid driver’s license and reliable transportation
• Excellent clinical, communication, and organizational skills
• Home health experience preferred, but not required
• Experience with WellSky or another home health EMR is a plus
