Your Role

We’re seeking a compassionate, experienced Home Health Registered Nurse (RN) to join our high-performing team. The ideal candidate has strong clinical judgement, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of skilled nursing care in the home setting.

What You’ll Do

• Conduct OASIS assessments and complete state-of-care, recertification, resumption-of-care, and discharge visits
• Develop and implement individualized plans of care in collaboration with physicians and the interdisciplinary team
• Perform comprehensive nursing assessments and ongoing patient evaluations
• Administer medications, perform skilled nursing procedures, and provide wound care as ordered
• Educate patients and caregivers on disease management, medications, safety precautions, and infection prevention
• Supervise and coordinate care provided by LPNs, HHAs, and other team members as appropriate
• Monitor patient progress, update care plans, and maintain thorough, timely, and compliant documentation
• Ensure compliance with all Medicare, CMS, and home health regulations

What We’re Looking For

• Current Florida Registered Nurse (RN) license (required)
• Previous home health experience is strongly preferred but not required
• OASIS experience preferred
• Excellent assessment, communication, time management, and documentation skills
• A compassionate, dependable, and professional attitude
