Job summary

CareRing Health, LLC seeks a registered nurse to perform initial and periodic home health assessments for admissions and ongoing care, including consideration of OASIS items when appropriate. The role emphasizes accurate nursing diagnoses, care plans, and physician-approved orders.

The RN will provide comprehensive nursing care in clients' homes, educate families on medications and procedures, monitor changes in status, and coordinate with the VP of Clinical Services and physicians to ensure

Qualifications

• RN must be licensed and a graduate from an approved nursing program.
• Two years of acute setting experience or demonstrated ability in heart, lung, and physical assessments.
• Current CPR certification required.
• Excellent communication, organizational and human-relations skills.
• Ability to work with minimal supervision and manage multiple priorities.
• Strong critical-thinking skills.

• Provides comprehensive nursing care through home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatments requiring physician-ordered specialized skills.
• Observes signs/symptoms and reports to VP Clinical Services/Director of Professional Services or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet as ordered by physician.
• Educates patients and families on medications and procedures.

Overview

The RN completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.

Responsibilities

• Accepts client admissions as assigned by Agency and weekend on-call assignments.
• Provides comprehensive nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatment(s) requiring specialized skills ordered by the physician.
• Observes signs and symptoms and reports them to the VP of Clinical Services / Director of Professional Services and/or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assess nutritional needs and diet of clients as ordered by physician.
• Educated patients families on medication and procedures.

Qualifications

• Holds a current Registered Nurse license
• Is a graduate from an approved school of nursing.
• Has a minimum of two years of experience in an acute setting that requires heart, lung, and physical assessments or has demonstrated ability or education in the same areas.
• Has current CPR certification.
• Has excellent communication, organizational, and human relations skills.
• Is able to function with minimal supervision.
• Is able to manage multiple projects/changing priorities.
• Is able to employ critical thinking skills.
