Job summary

Qualifications

Responsibilities

• Complete OASIS documentation efficiently and in compliance with agency policies.
• Assess patient needs and implement nursing interventions and medical plans of care.
• Collaborate with LPNs, delegating tasks to ensure adherence to care plans.
• Maintain infection control standards throughout the plan of care.
• Provide education to patients and caregivers on treatment options and discharge instructions.
• Document and evaluate patient responses and overall care provided.
• Coordinate safe patient discharges and transfers, ensuring all paperwork is processed.
• Participate in monthly peer review audits and case conferences for effective team communication.
• Advocate for patient care issues to ensure quality care tailored to individual needs.

Schedule/Shift

Monday to Friday, 8 AM to 4 PM, with some weekends, on-call, and holidays required.

Job Description

The Registered Nurse will utilize the nursing process to assess patient and caregiver needs, develop and communicate a realistic plan of care, manage case loads, and ensure quality care is delivered in accordance with evidence-based practices and regulatory standards.
