Job summary

Ontario Trillium Foundation in Bay Springs, MS is seeking a registered nurse to support home health clients. The role requires full-time hours with one weekend per month on call, and performing physician-directed plans of treatment while coordinating patient care with caregivers.

You will supervise Certified Nurse Aides per Medicare guidelines, participate in 24-hour RN availability, standby coverage, and maintain schedules that follow physician orders, ensuring high-quality, compliant home

Responsibilities

• Carry out Physician's Plan of Treatment and report changes as necessary
• Consult with physician regarding disease and symptom management needs
• Instruct and supervise the patient and/or the responsible person in meeting patient care needs
• Demonstrate the ability to observe, record and report needs/ concerns and coordinate the care of the patient while maintaining a constructive relationship with the patient and caregivers
• Provide supervisory visits for Certified Nurse Aides as per Medicare guidelines
• Participate in standby coverage to provide 24 hour RN availability to Home Health clients, will be on call a minimum of once a week and will be required to participate in call rotation on weekends, and holidays
• Develop a schedule for patient care and maintain a schedule that follows physician orders
• Perform related duties as assigned by supervisor
• Maintain compliance with all company policies and procedures
