Job summary

CareRing Health, LLC is seeking a Registered Nurse for field-based home health visits in the Washington, DC area. You will perform comprehensive patient assessments, develop and follow physician-approved care plans, and provide skilled nursing care in the patient’s home.

Responsibilities include admissions, weekend on-call, and collaboration with physicians and the VP of Clinical Services to monitor patient outcomes and safety. CPR indicates ongoing competency and strong communication abilities.

Qualifications

  • RN license is required and active.
  • Graduate of an approved school of nursing.
  • Minimum two years of acute care experience or equivalent in assessment.
  • Current CPR certification.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to function with minimal supervision and manage multiple priorities.
  • Strong critical thinking and problem-solving abilities.

  • Completes initial and periodic assessments for admission and ongoing care to determine needs and outcomes.
  • Accepts client admissions and weekend on-call assignments.
  • Provides comprehensive nursing care using home assessments, nursing diagnoses, and care plans.
  • Performs physician-ordered treatments requiring specialized skills.
  • Observes patient status and reports changes to leadership or physicians as appropriate.
  • Notes changes in physical or emotional status of clients.
  • Assesses nutritional needs and educates patients and families on medications and procedures.

Responsibilities

What You Will Do:

  • Completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.
  • Accepts client admissions as assigned by Agency and weekend on-call assignments.
  • Provides comprehensive nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
  • Performs treatment(s) requiring specialized skills ordered by the physician.
  • Observes signs and symptoms and reports them to the VP of Clinical Services / Director of Professional Services and/or physician as appropriate.
  • Notes changes in physical or emotional status of clients.
  • Assess nutritional needs and diet of clients as ordered by physician.
  • Educated patients families on medication and procedures.
Qualifications
  • Holds a current Registered Nurse license
  • Is a graduate from an approved school of nursing.
  • Has a minimum of two years of experience in an acute setting that requires heart, lung, and physical assessments or has demonstrated ability or education in the same areas.
  • Has current CPR certification.
  • Has excellent communication, organizational, and human relations skills.
  • Is able to function with minimal supervision.
  • Is able to manage multiple projects/changing priorities.
  • Is able to employ critical thinking skills.
