Job summary

CareRing Health is seeking a registered nurse for home health. You will perform comprehensive assessments for admission and ongoing care, coordinate with physicians, and provide nursing care using physician-approved plans.

Weekend on-call duties and admissions are part of the role. The ideal candidate holds an active RN license, has acute care experience and CPR certification, and demonstrates strong communication, organization, and critical thinking skills.

Qualifications

• Must hold current RN license.
• Graduate from an approved school of nursing.
• Minimum two years of acute setting experience with heart, lung, and physical assessments or equivalent education.
• Current CPR certification.
• Excellent communication, organizational and human relations skills.
• Able to function with minimal supervision.
• Able to manage multiple projects and changing priorities.
• Able to employ critical thinking skills.

Responsibilities

• Completes initial and periodic comprehensive assessments for admission and ongoing care to determine needs and outcomes.
• Accepts client admissions and weekend on-call assignments.
• Provides comprehensive nursing care using home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatments ordered by the physician that require specialized skills.
• Observes patient status and reports changes to supervisor/physician.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet as ordered by physician.
• Educates patients and families on medications and procedures.

Responsibilities

What You Will Do:

• Completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.
• Accepts client admissions as assigned by Agency and weekend on-call assignments.
• Provides comprehensive nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatment(s) requiring specialized skills ordered by the physician.
• Observes signs and symptoms and reports them to the VP of Clinical Services / Director of Professional Services and/or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet of clients as ordered by physician.
• Educated patients families on medication and procedures.

Qualifications
