Job summary

FirstHealth of the Carolinas seeks a Home Care Registered Nurse to manage clinical care for an assigned patient population in home health. You will develop patient-centered care plans with a multidisciplinary team and focus on achieving positive patient outcomes.

Qualifications

Responsibilities

• Manage clinical care for an assigned patient population in home health.
• Develop patient-centered, comprehensive care plans with multidisciplinary teams.
• Achieve positive patient outcomes and coordinate with the Clinical Manager.
• Supervise Home Health Aides and LPNs according to policy.
• Report daily to the Clinical Manager with pertinent patient information and maintain accurate documentation.
• Adhere to safety and infection control measures in all care activities.

Home Care RN Job Summary

• The RN Case Manager is responsible for the clinical management of an assigned patient population.
• The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach.
• The RN Case Manager is accountable for achieving positive patient outcomes.
• Supervises Home Health Aides and LPN’s according to policy.
• Reports daily to the Clinical Manager all pertinent patient information.
• Reports/records information accurately.
• Uses appropriate safety and infection control measures.

Home Care RN Qualifications

• Graduate of an accredited school of nursing with a current North Carolina RN license or RN multi‑state licensure privilege through the Nurse Licensure Compact.
• Minimum of one (1) year of acute care or home care experience required. Evidence of current CPR certification required.
• Valid North Carolina Driver’s License and proof of auto liability insurance required.
