Qualifications

• Graduate from an accredited school of nursing with a North Carolina RN license.
• Minimum of one year of acute or home health care experience; CPR certification required.
• Valid North Carolina driver’s license and proof of auto liability insurance required.

Responsibilities

• Manage clinical care for an assigned patient population in home health.
• Develop patient-centered, comprehensive care plans with multidisciplinary teams.
• Achieve positive patient outcomes and coordinate with the Clinical Manager.
• Supervise Home Health Aides and LPNs according to policy.
• Report daily to the Clinical Manager with pertinent patient information and maintain accurate documentation.
• Adhere to safety and infection control measures in all care activities.
