Job summary

FirstHealth of the Carolinas is seeking a Home Care RN Case Manager to oversee the clinical management of an assigned patient population within our home health program. You will develop a patient-centered, comprehensive plan of care using a multidisciplinary team approach to support quality outcomes.

You will supervise Home Health Aides and LPNs per policy, report daily to the Clinical Manager with pertinent patient information, and ensure accurate documentation while applying appropriate safety

Qualifications

• Graduate of an accredited school of nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact.
• Minimum of one (1) year of acute care or home care experience required. Evidence of current CPR certification required.
• Valid North Carolina Driver’s License and proof of auto liability insurance required.

Responsibilities

• The RN Case Manager is responsible for the clinical management of an assigned patient population.
• The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach.
• The RN Case Manager is accountable for achieving positive patient outcomes
• Supervises Home Health Aides and LPN’s according to policy
• Reports daily to the Clinical Manager all pertinent patient information
• Reports/records information accurately
• Uses appropriate safety and infection control measures
