PruittHealth @ Home is hiring a compassionate Registered Nurse (RN) for Home Health in North Carolina. As a Home Health RN Case Manager, you will deliver skilled nursing visits in patients' homes and develop start-of-care assessments and care plans.

This role offers flexible days, a competitive pay range based on experience ($74,880 - $90,480), opportunities for training, merit increases, and strong clinical support from a dedicated team.

Qualifications

• Active, unrestricted Registered Nurse (RN) license.
• Valid driver’s license & reliable transportation.
• CPR certification.
• 1+ year of RN experience (home health or acute care preferred).

Responsabilités

• Provide skilled nursing visits in the patient’s home.
• Complete assessments, start-of-care visits, and care plans.
• Collaborate with the interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assist (OASIS experience a plus).
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.

Monday - Friday, Flexible Dayshift Hours

Coverage Area: Dobson, Flat Rock, Mount Airy, Pilot Mountain, Toast, White Plains, Danbury, Germanton, Pinnaclie, Walnut Cover and surrounding communities throughout Surry and Stokes Counties.
