Job summary

Talented Medical Solutions is seeking a Registered Nurse for home care in Mesa, AZ. The RN will provide skilled nursing services in patients' homes, manage chronic and acute conditions, and promote patient independence and safety.

The role includes conducting comprehensive assessments, developing individualized care plans with physicians, administering medications and wound care, and educating patients and caregivers on disease management.

Qualifications

• Associate's or Bachelor's Degree in Nursing (Required).
• Current RN license in state of practice (Required).
• BLS certification (Required).

Responsibilities

• Conducts comprehensive in-home patient assessments, including OASIS documentation as applicable.
• Develops and updates individualized plans of care in collaboration with physicians.
• Administers medications, wound care, and other skilled nursing interventions in the home setting.
• Educates patients and caregivers on disease management, medications, and safety.
• Coordinates care with physical therapy, social work, and other home health team members.
• Documents visits, assessments, and patient progress in compliance with regulatory requirements.

Education & Experience

Licenses & Certifications

• Basic Life Support (BLS) certification (Required)
• Valid driver's license and reliable transportation (Required)
