Job summary

Talented-Medical-Solutions is seeking a skilled RN for on-site home health visits in Tucson, AZ. You will provide care per plan and physician orders, assess needs, monitor vitals, and document accurately.

You will educate patients and families, adjust care plans with supervisors and physicians, and collaborate with the care team to ensure safe transitions and continuity of care. This role emphasizes reliability, productivity, and adherence to infection control and quality improvement

Qualifications

• Licensed Registered Nurse (RN) with current state nursing license.
• Experience in home health care or skilled nursing visits.
• Ability to assess vitals, monitor changes, and document accurately in EMR systems.
• Certified Home Health Nurse (CHHN) or similar home health certification preferred.
• Active CPR/First Aid certification and emergency preparedness skills.
• Strong documentation skills with attention to compliance and care plans.
• Excellent communication with patients, families, and care team; educational skills.
• Ability to develop and adjust care plans collaboratively; participate in QA initiatives.
• High accountability for performance and professional growth; self-motivated learner.
• Reliable time management and routing efficiency to meet visit targets.
