Qualifications

• Licensed RN with current state license.
• Experience in home health or skilled nursing visits.
• Ability to assess vitals, document accurately in EMR.
• Strong time management and routing efficiency.
• CHHN or similar home health certification preferred.
• Active CPR/First Aid certification.
• Strong communication with patients and care team.
• Ability to develop and adjust care plans collaboratively; participate in QA initiatives.
• High accountability for performance and professional growth.
• Participation in team meetings and quality improvement initiatives.
• Identify and report supply needs and equipment issues.
• Support transition of patients between care settings to maintain continuity.

Uphold infection control and safety standards in all home care settings.

Educate patients and family on care procedures, medications, and safety practices.

Experience/Qualifications

Licensed Registered Nurse (RN) with current state nursing license.

Proven experience in home health care or skilled nursing visits (X+ months).

Ability to assess vitals, monitor changes, and document accurately in EMR systems.

Reliable time management and routing efficiency to meet visit targets.

Certified Home Health Nurse (CHHN) or similar home health certification preferred.

Active CPR/First Aid certification and emergency preparedness skills.

Strong documentation skills with attention to compliance and care plans.

Excellent communication with patients, families, and care team; educational skills.

Ability to develop and adjust care plans collaboratively; participate in QA initiatives.
