Job summary

Mercy Home Health is hiring a Registered Nurse to provide day-to-day in-home patient care across the St. Louis metro area. You’ll perform assessments, manage post-hospitalization plans, and coordinate with an interdisciplinary team to ensure safe, high-quality care in patients’ homes.

Responsibilities include case management, documentation in Epic, and adherence to Mercy Service Standards. We offer full- and part-time day shifts, mileage reimbursement, and a supportive, mission-driven

Qualifications

• One year of experience in home health, acute care, or related field.
• Graduation from an accredited nursing program.
• Current RN license in Missouri.
• Valid driver's license and proof of auto insurance.
• BLS Certification through the American Heart Association.

Responsibilities

• Provide day-to-day patient care in the home setting with varied acuity.
• Perform post-hospitalization assessments and care coordination.
• Complete documentation using the Epic system.
• Assist with admissions, recertifications, discharges, and patient teaching.
• Adhere to Mercy Service Standards and policies.

Job Profile Summary

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Qualifications

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).
• Education: Graduation from an accredited nursing program required.

• Certifications: BLS Certification required through the American Heart Association.
