Job summary

Mercy Home Health seeks a dedicated RN to deliver high-quality, patient-centered care in patients’ homes across the St. Louis Metro and surrounding counties. The role involves managing care plans, performing post-hospital assessments, and ensuring safety and comfort for diverse patient populations.

Flexible Full-Time and Part-Time Day Shift opportunities are available, with Per Diem visits as needed. A mileage reimbursement program and Mercy Service Standards guide daily practice.

Qualifications

• Must be a graduate of an accredited nursing program.
• RN license in Missouri is required.
• BLS certification through American Heart Association is required.

Responsibilities

• Provide day-to-day patient care in the home setting with varying acuity.
• Perform case management and total care for multiple patients.
• Complete timely documentation using Epic EMR.

Job Profile Summary

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.
Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Qualifications:

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).
• Education: Graduation from an accredited nursing program required.
• Licensure: Current RN license in the state of Missouri required. Must also have a valid driver's license and proof of auto insurance.
• Certifications: BLS Certification required through the American Heart Association .
