Job summary

Cleveland Clinic Home Care Services is seeking an RN Homecare Float to deliver direct nursing care in patients’ homes and to conduct supervisory visits as needed in the West-Center for Connected Care region.

The role is based in Independence, Ohio, covering the West of I-77 service area with evenings shifts; BSN preferred and a strong focus on quality improvement and adherence to policy and protocol.

Qualifications

• Graduate of an accredited school of nursing.

Responsibilities

• Provides direct nursing care in accordance with established policies, procedures and protocols.
• Supervises starts of care, resumptions of care and routine visits as requested.
• Assesses patient safety risks and documents findings.
• Coordinates care with other disciplines to ensure well-coordinated patient care.
• Documents pain assessments and manages pain in collaboration with patient and health care team.
