Job summary

Cleveland Clinic is seeking a PRN Home Health RN to deliver patient-centered nursing care in patients' homes within the West of I-77 service area. You will coordinate care, assess pain, and ensure safety while collaborating with families and the health team.

Responsibilities include supervising care staff, documenting care plans, and escalating changes to physicians as appropriate. Per diem, day shift; mileage reimbursement and flexible scheduling are offered.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• RN license in Ohio (active).
• BLS certification through the American Heart Association.
• BSN preferred but not required.

Responsibilities

• Provide direct nursing care per organization policies and protocols.
• Implement and monitor patient care plans and care coordination.
• Supervise LPNs, Home Care Aides, and families involved in care.
• Perform admission assessments and ongoing reassessments as needed.
• Document patient condition, interventions, and outcomes timely.
