Qualifications

• RN license is required.
• Associate's degree in Nursing preferred, with 2–5 years of relevant experience; 5–10 years preferred for advanced roles.
• Experience in home care is a plus.
• Ability to multitask, prioritize, and travel between hospital locations.

Responsibilities

• Assist in discharge planning and coordination of care activities.
• Participate in planning and educating about home health services and specialty programs.
• Serve as a liaison between internal teams and external partners.
• Document all care coordination efforts accurately.
• Contribute to sales and marketing efforts through education and outreach.
