LiveWell Homecare Agency is seeking a full-time, on-site RN role based at a hospital location in St. Louis, MO, with travel between sites and weekend/holiday/on-call rotations.

The coordinator will act as a liaison among patients, physicians, hospital staff, and home health agencies, contributing to discharge planning, documenting care coordination activities, and educating staff and the community about home care options and programs.

Qualifications

• RN license required (Nursing Diploma or Associate's degree in Nursing).
• 2-5 years minimum experience; 5-10 years preferred for advanced roles.
• Strong communication, coordination, and educational skills; ability to educate staff and community.

Responsibilities

• Facilitate patient discharge planning and care coordination.
• Maintain thorough documentation of all activities.
• Participate in planning and marketing of home health services.
• Educate staff and community about home care options and programs.
• Coordinate across patients, physicians, hospital staff, and home health agencies.
