Job summary

LiveWell Homecare Agency, LLC is seeking an experienced RN to provide direct nursing care in a homecare setting, following established policies and protocols. You will implement and monitor patient care plans and serve as a primary coordinator among disciplines for well-coordinated care.

You will also follow physician and nursing orders and assess discharge planning needs with the healthcare team to ensure smooth transitions and optimal outcomes for patients in Alabama.

Qualifications

• Current or eligible RN licensure by the Alabama Board of Nursing.
• BLS certification within 30 days of hire from AHA or Red Cross.
• Diploma from an accredited nursing school and at least one year of RN experience.

Responsibilities

• Provides direct nursing care following established policies, procedures, and protocols.
• Implements and monitors patient care plans, communicates patient conditions, and serves as a primary coordinator among disciplines for well-coordinated care.
