Job summary

Washington Regional Medical System in Fayetteville, AR is seeking an experienced RN Home Care Coordinator to manage the PACE program, ensure regulatory compliance, and promote high-quality care for participants in the community.

You will conduct initial health assessments, contribute to care plans, educate families, supervise in-home caregivers, and collaborate with an interdisciplinary team to optimize resource utilization and patient outcomes.

Qualifications

• Must be a graduate of an accredited nursing program.
• Registered Nurse license and CPR are required.
• Minimum of 3 years of nursing experience; elderly population preferred.

Responsibilities

• Assess home care needs and develop care plans for elderly clients.
• Coordinate durable medical equipment and services as needed.
• Participate in interdisciplinary care team meetings.
• Manage home care staffing resources to match participant needs.
• Train and supervise in-home caregiver staff.
• Communicate with participants and families about home care needs.
• Document home care records and ensure billing accuracy.
• Maintain equipment, supplies, and services per care plan.
• Provide in-home treatments and periodic reassessments.
• Establish relationships with program staff and community partners.
