EvergreenHealth’s Heart Care Clinic in Kirkland, WA is seeking an experienced Registered Nurse to provide direct patient care, triage, and education. The role emphasizes coordinating care across the clinic, hospital, ER, and other health services.

This position will transition to the Call and Referral team by January 2027, supporting cardiovascular patients while embedded in the team, with emphasis on collaboration and efficient resource use.

Qualifications

• Graduate of a nursing program (Diploma/ADN/BSN/MSN) required.
• Licensed RN in WA or multi-state NLC license.
• 3 years RN experience.
• BLS – American Heart Association current by start date.
• ACLS certification current by date of hire.
• Chronic care management and patient education skills.

Responsibilities

• Manages clinic flow and ensures continuity of care between clinic and hospital/system.
• Triages patients in person and by phone.
• Assesses nursing needs and develops nursing diagnoses.
• Collaborates with providers to implement the plan of care.

Wage Range: $45.19 - $82.30 per hour

Job Summary

EvergreenHealth’s Heart Care Clinic is looking for an experienced RN to join our well-established team. This position will provide nursing care through face-to-face or phone, by process of analyzing information delivered by the patient to sort out the need of appropriate care related to the severity of the concerns and symptoms represented. Provides patient education as needed for better care and compliance.

Ensures the direct nursing care of patients is conducted within established standards of care, hospital and unit policies, procedures, and guidelines. May assist in the developing of case management protocols.

We anticipate this position transitioning to our Call and Referral team by January 2027. This role will continue to support Heart Care by performing symptom triage for cardiovascular patients while imbedded in our Call and Referral team.

Primary Duties

• 1. Manages the clinic flow.
• 2. Provides continuity of care between the clinic, hospital, ER, specialists, Home Health, Hospice and LTC facilities.
• 3. Triages patients in person and via phone.
• 4. Assesses the comprehensive nursing needs and develops a nursing diagnosis.
• 5. Participates with the providers to develop and implement the plan of care.
• 6. Analyzes diagnostic / lab test results and relays that information to the providers.
• 7. Coordinates critical results in a timely manner.
• 8. Provides oversight of medical assistants to ensure patient care is provided within established guidelines and evaluates the outcomes of care.
• 9. May assist with the Medical Home pilot by attending collaborative meetings, preparing month end reports, and working toward case management of patients in conjunction with the nurse case manager.
• 10. Performs nursing visits within scope of practice and assists with developing group visits with manager and providers.
• 11. Provides education and training for medical assistants in collaboration with the manager and/or RN Quality Manager.
• 12. Balances workloads and adjusts staffing in various pods to adequately cover providers.
• 13. Provides safe, family centered nursing patient care and serves as a consultant to the patients and caregivers.
• 14. Performs other duties as assigned.

REQUIRED for the position

• Graduate of a nursing program (Nursing Diploma, Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN) degree and/or Master of Science in Nursing (MSN) degree)
• Licensed as a Registered Nurse in the State of Washington OR holding a multistate Nurse Licensure Compact (NLC) license
• 3 years of previous RN experience
• Chronic care management and patient education skills
• Ability to manage a variety of concurrent programs and projects effectively while Utilizing time, equipment and supplies appropriately
• Current Healthcare Provider BLS – American Heart Association by start date
• Current Healthcare Provider ACLS certification by date of hire

DESIRED for the position

• Previous lead and/or supervisory experience demonstrating effective, positive and constructive feedback skills
• Master’s in Nursing (MSN)
• Recent ICU/CCU/PCU/Cath Lab experience
