What if your nursing role gave you more time to educate, coach, and help patients make lasting changes—not just address what's happening in the moment? At CareATC, our employer-sponsored primary care model gives patients greater access to care and gives our clinical teams the opportunity to focus on prevention, relationships, and better long-term health. As an RN Health Coach/Diabetes Educator, you'll partner with our primary care team to support patients managing diabetes and related chronic conditions. You'll have the opportunity to build ongoing relationships, provide meaningful education, and help patients turn clinical recommendations into realistic changes they can sustain.
The RN Health Coach/Diabetes Educator works closely with our primary care team to help patients better understand and manage their health. You'll receive referrals from providers and work primarily with patients managing diabetes and related comorbidities such as obesity, hypertension, and cardiovascular risk factors. You'll combine your nursing expertise with education, motivational interviewing, and individualized goal setting to help patients build the knowledge and confidence to manage their health successfully. Beyond one-on-one coaching, you'll have opportunities to support broader wellness and engagement efforts through educational presentations, webinars, and onsite events.
If you're an RN who loves the education and relationship-building side of nursing and wants to help patients take greater control of their health, this could be the opportunity you've been looking for. Bring your clinical expertise to CareATC and help patients build healthier lives—one conversation, one goal, and one lasting change at a time.
