Mainline Health Systems, Inc. is seeking a Health Coach (RN) in Monticello, AR to support diabetes and cardiometabolic care under the Director of Health Education.
You will work with the Diabetes Self-Management team to optimize patient outcomes through education and care coordination.

8 a.m. to 5:30 pm Monday- Thursday.
8 a.m. to 12:00 pm on Friday.
However, additional hours may be required to complete duties.
The Health Coach is part of the care team andunder the supervision of the Director of Health Education. The Health Coach will work in conjunction with the Diabetes Self-Management team to assist the care team with direction and resources regarding diabetes self-management aimed at meeting the needs of the patients. The health coach will integrate clinical and self-management aspects of diabetes and cardiometabolic care in order to optimize quality care. The health coach promotes self-management to achieve individualized behavioral and treatment goals. The Health Coach will assist in meeting organizational goals. The Health Coach is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.
The Health Coach reports to and is supervised and evaluated by the Director of Health Education.

The evaluation of work performance will be ongoing, and will be carried out by the Director of Health Education. It will include specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will be performed annually, but may be initiated at other times by the Director of Health Education.
