Job summary

HealthCare Strategies is seeking a remote Care Manager to educate and coach patients with chronic conditions. You will work from home on a Monday–Friday core schedule, delivering telephonic coaching to improve outcomes and reduce costs. The role requires an active RN license in the US and may involve Spanish-speaking patients.

You will identify eligible patients, enroll them in the program, and document all interactions. A supportive team and competitive benefits accompany this opportunity.

Qualifications

• RN license required, active and unrestricted.
• Experience in employee wellness or health promotion preferred.
• Spanish-speaking ability is a plus.

Responsibilities

About the Role

As a Care Manager, you’ll provide telephonic education and coaching to patients managing chronic conditions such as diabetes, hypertension, and obesity. You’ll be part of a supportive program designed to improve patient outcomes and lower healthcare costs — and you’ll be trained in health coaching methods to help patients make positive lifestyle changes.

Key Responsibilities

What We’re Looking For

Education & Experience
