Islands Skilled Nursing and Rehabilitation, located in Honolulu, HI, seeks Registered Nurses for full-time and part-time roles in a 42-bed skilled nursing and rehabilitation community specializing in rehab, ventilator/tracheostomy care, and post-acute services.

You will contribute to compassionate resident care, pursue career growth opportunities including tuition reimbursement and advancement courses, and work with an interdisciplinary team to promote quality outcomes for Islanders.

Qualifications

• Current active RN license in the state of practice.
• RN clinical assessment skills as required for the job.
• Ability to make independent decisions when circumstances warrant such action.
• Knowledge of nursing and medical practices, and laws relating to long-term care.
• Effective interpersonal skills and ability to work with an interdisciplinary care team.
• Ability to plan, organize, develop, implement, and interpret programs, goals, policies, and procedures for quality care.
• Capable of researching new methods and incorporating them into nursing practice.
• Understanding of computer technology.
• Proven written and oral communication skills.
• Proven decision-making and analytical skills.
• Ability to relate positively with residents, families, and external agencies.
• Recent clinical experience; geriatrics preferred; LTC experience 1 year preferred.
• Learn to work with vent/trach patients.

Responsibilities

• Provide RN clinical assessments and develop care plans.
• Deliver resident-centered care in collaboration with the care team.
• Monitor and document patient status and changes.
• Assist with vent/trach patients and post-acute care needs.
• Ensure compliance with regulations and best practices in long-term care.
