Job summary

Atrium Health Jesse Helms Nursing Center in Monroe, NC seeks an experienced RN to manage an assigned patient caseload, perform medication administration, and document to meet care plans and physician orders.

You will collect data from assessments, identify high-risk patients, coordinate care with LPNs and HHAs, participate in admissions/discharges, and support the clinical team to ensure safe, high-quality outcomes.

Qualifications

• RN license required to practice in state of employment.
• BSN preferred for clinical leadership and care coordination.
• BLS certification as per policy guidelines.

Responsibilities

• Manage daily patient caseload with timely care and documentation per plan and MD orders.
• Collect data from assessments and EMR to identify patient needs and risks.
• Analyze information to determine issues and resources, mitigate re-hospitalization risk.
• Prioritize multi-issue situations ensuring patient safety and timely services.
• Develop and communicate a plan of care to all involved parties.
• Coordinate care with a multidisciplinary team; supervise LPNs and HHAs as needed.
• Perform ordered treatments and procedures in line with regulations and policies.
• Evaluate progress toward patient-centered goals.
