SolutionHealth's Foundation Gastroenterology in Nashua, NH is seeking an Ambulatory Care Nurse to coordinate care for complex GI patients, educate on disease management, and support transitions of care.

The ideal candidate holds ASN/BSN, NH RN license, BLS/CPR, and 2+ years of nursing with ambulatory or care coordination experience; EMR proficiency is a plus. We offer flexible scheduling and development opportunities.

Qualifications

• Experience in ambulatory care or care coordination is preferred.
• Strong clinical assessment and communication skills are required.
• Ability to work with patients, families and care teams to develop care plans.
• Proven ability to manage multiple priorities and work independently.

Responsibilities

• Provide care coordination and case management for GI patients.
• Collaborate with providers and interdisciplinary teams to develop individualized plans.
• Educate patients on disease management, medications, nutrition, and self-care.
• Coordinate transitions of care including hospital discharges and referrals.
• Utilize EMR tools to identify and support patients needing outreach and follow-up.
• Promote safety, quality outcomes, and continuity of care.
