Job summary

Foundation Gastroenterology, affiliated with Southern New Hampshire Medical Center, is seeking a part-time RN to support a high‑quality digestive care team in Nashua, NH. You will assist providers, triage, and deliver patient-centered care across in-person, telephone, and telehealth channels.

You’ll collaborate with physicians, PAs, and other health professionals to design and implement care plans, administer medications, perform point-of-care testing, and educate patients on self-management and

Qualifications

• Current NH Registered Nursing (RN) licensure is required.
• Certifications: Current BLS/CPR training.
• Prior work experience in a healthcare setting is required.
• Epic experience is a plus.

Responsibilities

• Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.
• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities
• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
• Administrate prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
