Job summary

Southern New Hampshire Health's Foundation Gastroenterology is seeking an experienced registered nurse to manage care for complex GI patients. You will coordinate education, self-management support, and care transitions across a multidisciplinary team.

The role requires an NH RN license, BLS/CPR, and at least two years of nursing experience, with ambulatory or care coordination background preferred. 4 days/week, 8a-5p, with comprehensive benefits.

Qualifications

• ASN or BSN required/preferred; NH RN license required.
• BLS/CPR certification required; 2+ years nursing experience.
• Ambulatory care or care coordination experience preferred.

Responsibilities

• Provide care coordination and case management for gastroenterology patients.
• Collaborate with providers and care teams to create individualized plans.
• Educate patients on disease management, meds, nutrition, and self-care.
• Coordinate transitions of care, discharges, referrals, and follow-up.
• Utilize EMR and population health tools to identify patients needing support.
