Job summary

Foundation Gastroenterology in Nashua, NH, part of SolutionHealth, seeks an experienced RN to manage care for complex gastroenterology patients, promoting education, self-management support, and timely care delivery. You will work with patients and interdisciplinary teams to develop individualized care plans, coordinate transitions, and use the EMR to identify patients needing extra support.

Qualifications

• ASN or equivalent from an accredited institution is required.
• BSN degree is preferred.
• Current NH RN license and BLS/CPR certification required.
• Minimum of two years of professional nursing experience.
• Ambulatory care or care coordination experience preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide care coordination and case management for complex gastroenterology patients.
• Collaborate with providers and interdisciplinary care teams to develop individualized care plans.
• Educate patients on disease management, medications, nutrition, and self-management strategies.
• Coordinate transitions of care including hospital discharges and referrals.
• Utilize EMR tools to identify and manage patients needing additional support.
• Promote patient safety, quality outcomes, and continuity of care.
