This position is full-time with a shift requirement of 7:00PM-7:00AM. W eekend, and holiday availability required.

ESSENTIAL FUNCTIONS

• Follows policies and practices to ensure consistency with current standards promulgated by appropriate local, state, and federal regulatory bodies and within limits of the respective State Nurse Practice Act.
• Is competent in acting in emergency situations based on Safety Manual, Internal and External Emergency Preparedness and other policies.
• Complies with Mather Code of Conduct, Policies, Compliance Program, and all applicable laws and regulations in performance of one’s job duties; reports all known and suspected violations to his/her supervisor or manager, Mather Compliance officer, a Human Resources representative, or the confidential employee helpline.
• Identifies areas for improvement, proposes possible solutions, and participates in data collection and process changes as part of the Quality Assurance Process Improvement process.
• Assesses each resident’s physical, psychosocial, environmental, spiritual, cultural, educational and discharge planning needs, as appropriate.
• Participates in the establishment of desired outcomes with resident, family, and interdisciplinary team.
• Participates in the development of a care plan/service plan.
• Delegates tasks based on resident need and according to knowledge and skill of the designated caregiver.
• Performs personal care for residents, as needed, i.e. toileting, bathing, etc.
• Documents nursing care per documentation policy and standards.
• Applies the concepts of the Mather Wellness Model and the 3 A’s in the care of residents, Achievement, Autonomy, and Affiliation.
• Assists in licensing and survey preparedness and processes.
• Uses a customer-focused approach to problem solving philosophy.
• Maintains a courteous and professional manner through interactions with others.
• Communicates with residents and families in a timely manner.
• Uses discretion in handling confidential information, incorporating all Federal, State and local privacy and confidentiality requirements.
• Provides input for all relevant human resource functions including but not limited to: hiring, supervision, evaluation, staff development, scheduling, discipline, and termination of staff.
• Makes recommendation for hiring and retention.
• Participates in resident and family education and counseling.
• Interactions with employees, residents, resident representatives, and guests will reflect our commitment to inclusivity.

QUALIFICATIONS AND SKILLS

Required:

• Current state RN license.

Preferred:

• 1+ years experience in skilled Nursing setting.
• Supervisory/leadership experience.
• Basic computer knowledge.

The hourly base wage range provided is a good faith estimate of what Mather reasonably expects to pay for this position at the time of the posting. The actual hourly rate offered will be determined based on a variety of factors, including but not limited to relevant skills, certifications, years of experience, geographic location, and current market conditions. Eligibility for benefits is dependent on employment status and hours worked per pay period. Hourly Pay Range: $42 USD - $48 USD
